Video-assisted thoracoscopy is a safe and minimally invasive diagnostic and treatment option for pleural and lung diseases. Anesthesia management, including general anesthesia and local anesthesia with sedation, was evaluated in 82 procedures, with low complication rates.

Area of Science:

  • Thoracic Surgery
  • Anesthesiology
  • Pulmonology

Background:

  • Thoracoscopy is increasingly utilized for diagnosing and treating pleural and lung conditions.
  • Technical advancements are driving the rise in thoracoscopic procedures.

Purpose of the Study:

  • To describe anesthesia management experiences in thoracoscopic procedures.
  • To evaluate the safety and efficacy of different anesthesia techniques in thoracoscopy.

Main Methods:

  • Review of 82 thoracoscopic procedures, including 48 video-assisted thoracoscopies.
  • Comparison of balanced general anesthesia (GA) with isoflurane, fentanyl, and atracurium (62 cases) versus local anesthesia (LA) with propofol and fentanyl (20 cases).
  • Selective bronchial intubation was performed in 46 cases.

Related Experiment Videos

Main Results:

  • Local anesthesia patients were significantly older (63 +/- 17 years) than general anesthesia patients (49 +/- 13 years).
  • ASA IV patients received LA and sedation.
  • Complications included severe hypoxemia (8 cases), moderate hypoxemia (1 case), coughing (3 cases), and agitation (1 case).

Conclusions:

  • Video-thoracoscopy is a safe and less invasive alternative to conventional thoracotomy.
  • The application of thoracoscopy is expanding, but its definitive role in thoracic surgery is yet to be established.
