Diagnostic vitrectomy aids in diagnosing chronic uveitis when other methods fail. This procedure can identify infectious or tumoral causes and improve vision in about half of the patients.

Area of Science:

  • Ophthalmology
  • Surgical Pathology

Background:

  • Vitrectomy, initially used for infectious endophthalmitis, is also valuable for chronic uveitis diagnosis and treatment.
  • Unexplained chronic uveitis cases pose diagnostic challenges.

Purpose of the Study:

  • To evaluate the diagnostic and therapeutic utility of vitrectomy in chronic uveitis of unknown etiology.

Main Methods:

  • Vitrectomy was performed on 28 eyes of 25 patients with chronic uveitis unresponsive to corticosteroid treatment.
  • Patients had no previously identified cause for their condition.

Main Results:

  • Vitrectomy directly led to a diagnosis in 9 eyes (32%), revealing infectious (bacterial, fungal, viral) and tumoral (lymphoma, melanoma metastasis) pathologies.

Related Experiment Videos

  • Fundus visualization during surgery identified additional diagnoses in 4 patients (retinal necrosis, Toxocara canis).
  • Overall diagnosis was achieved in approximately 50% of cases, with vision improvement also noted in half the patients.
  • Conclusions:

    • Vitrectomy is an effective tool for diagnosing chronic uveitis when etiology is unclear, directly diagnosing in about one-third of cases.
    • Intraoperative fundus findings significantly increase the diagnostic yield, reaching about 50% overall.
    • The procedure offers both diagnostic and visual prognostic benefits for patients with chronic uveitis.
