Summary

Calciphylaxis, a rare vascular disease, can present microscopic Pseudoxanthoma elasticum (PXE)-like changes in patients with chronic kidney disease. These PXE-like findings may occur without systemic PXE manifestations.

Background:

  • Calciphylaxis is a rare, severe condition characterized by vascular calcification, thrombosis, and necrosis, often linked to chronic renal failure and hyperparathyroidism.
  • Pseudoxanthoma elasticum (PXE) is a genetic disorder affecting elastic tissues, leading to skin, retinal, and visceral abnormalities.

Observation:

  • Histologic findings typical of PXE were observed in a patient with chronic renal failure who developed fatal calciphylaxis.
  • Comprehensive evaluation did not reveal systemic signs or classic clinical manifestations of PXE in this patient.

Findings:

  • The study suggests that rapid soft tissue calcification in calciphylaxis can induce histopathologic changes resembling PXE.
  • These PXE-like changes may appear as coincidental findings in calciphylaxis without underlying systemic PXE.
