Background:

  • Managing refractory depression in patients with severe medical comorbidities, particularly after major cardiac surgery, presents significant clinical challenges.

Observation:

  • A patient with refractory depression underwent a full course of ECT two weeks after a four-vessel CABG procedure.
  • The patient tolerated the ECT treatment well, with no apparent adverse cardiac or neurological events related to the therapy.

Findings:

  • The patient achieved a complete remission of depressive symptoms following the course of ECT.
  • This positive outcome suggests the feasibility and efficacy of ECT in this high-risk patient population.

Implications:

  • Electroconvulsive therapy (ECT) may be a viable and safe treatment option for refractory depression in patients with recent CABG surgery and significant medical comorbidities.
  • Further research is warranted to establish specific guidelines for ECT administration in the perioperative cardiac surgery setting.
