Summary

A cumulative index (CI) can help differentiate indeterminate leprosy from paucibacillary leprosy. Higher CI in lepromin-negative indeterminate cases may predict evolution to multibacillary leprosy.

Optimized Protocols for Mycobacterium leprae Strain Management: Frozen Stock Preservation and Maintenance in Athymic Nude Mice

Background:

  • Leprosy classification is crucial for treatment.
  • Distinguishing indeterminate leprosy (IND) from paucibacillary (PB) leprosy and predicting progression to multibacillary (MB) leprosy remain challenges.

Purpose of the Study:

  • To differentiate IND leprosy from PB leprosy.
  • To identify IND leprosy cases that may evolve to MB leprosy.
  • To evaluate serological, immunological, and histochemical parameters.

Main Methods:

  • Screened 92 untreated patients (TT, BT, IND, NSD) for antibodies against PGL-I, 35-kDa, and LAM antigens.
  • Performed lepromin tests.
  • Used indirect immunoperoxidase staining for antigen demonstration.

Main Results:

  • Quantitative analysis using a cumulative index (CI) showed higher antibody levels in IND patients compared to other PB groups.
  • Lepromin-negative IND patients had a higher CI than lepromin-positive IND patients.
  • 64% of nonspecific dermatitis (NSD) cases showed leprosy-specific antibodies or antigens, suggesting they were likely leprosy.

Conclusions:

  • A cumulative index (CI) may aid in differentiating IND from PB leprosy.
  • Higher CI in lepromin-negative IND patients could predict progression to MB leprosy.
  • Serological markers show promise in diagnosing and predicting leprosy progression.
