Summary

Gustatory sweating, a rare symptom in diabetes mellitus, is strongly linked to diabetic nephropathy and neuropathy. This study reveals it is more common than previously thought, often improving after kidney transplants.

Background:

Purpose of the Study:

  • To investigate the relationship between gustatory sweating and other diabetic complications, specifically nephropathy and neuropathy.
  • To determine the prevalence of gustatory sweating in patients with diabetic nephropathy and neuropathy.

Main Methods:

  • 196 patients were divided into four groups: diabetic nephropathy, diabetic neuropathy, diabetic controls, and non-diabetic renal failure.
  • Patients were questioned about gustatory sweating; somatic and autonomic neuropathy were assessed using clinical signs, vibration perception threshold, and heart rate variability.

  • 69% of patients with diabetic nephropathy and 36% with diabetic neuropathy reported gustatory sweating, compared to less than 5% in control groups.
  • A strong correlation was found between gustatory sweating and the degree of neuropathy (p < 0.01).
  • Five subjects experienced improvement or resolution of gustatory sweating post-renal transplantation.

Conclusions:

  • Gustatory sweating is significantly more common in patients with diabetic nephropathy and neuropathy than previously believed.
  • Gustatory sweating is closely associated with diabetic nephropathy and neuropathy, suggesting a shared pathophysiological basis.
