Summary
This study found that 6g daily of 4-aminosalicylic acid (4-ASA) improved mild to moderate ulcerative colitis extending beyond 60 cm. However, 4-ASA showed no benefit for distal colitis and was generally well tolerated.
Area of Science:
- Gastroenterology
- Clinical Pharmacology
- Inflammatory Bowel Disease Research
Background:
- Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
- Mild to moderate UC requires effective and safe treatment options.
- The efficacy of 4-aminosalicylic acid (4-ASA) varies based on disease extent.
Purpose of the Study:
- To evaluate the efficacy and safety of 6 g/day of 4-aminosalicylic acid (4-ASA) in mild to moderate ulcerative colitis.
- To compare 4-ASA efficacy between patients with distal (< 60 cm) and extensive (> 60 cm) disease.
- To assess the safety profile of 4-ASA in this patient population.
Main Methods:
- A six-week, placebo-controlled trial involving 30 subjects with mild to moderate ulcerative colitis.
- Patients were stratified based on disease extent: distal (< 60 cm) or extensive (> 60 cm).
- Efficacy was assessed using scores for diarrhea, bleeding, endoscopic appearance, biopsy results, and physician global assessment. Safety was monitored via symptoms and laboratory values.
Main Results:
- 4-ASA demonstrated significantly greater median percent improvement (42.7%) compared to placebo (21.2%) in the extensive disease group (P < 0.05).
- No significant difference in efficacy was observed between 4-ASA and placebo for the distal disease group or the total study cohort.
- Adverse events included severe dyspepsia in one subject, transient or persistent abnormal AST levels in six subjects, and elevated lipase without pancreatitis in six subjects.
Conclusions:
- Six weeks of 6 g/day 4-ASA is more effective than placebo for mild to moderate ulcerative colitis with extensive disease (> 60 cm).
- 4-ASA did not show superior efficacy compared to placebo for distal ulcerative colitis (< 60 cm).
- The treatment was generally well tolerated, with manageable adverse events observed.
