Summary

Laparoscopic myolysis effectively treated symptomatic uterine fibroids, significantly reducing uterine volume and obviating major surgery in most women. Fertility was preserved in select cases, but further studies are needed for broader application.

Background:

  • Symptomatic uterine leiomyomata (fibroids) are common and can cause menorrhagia, pain, and pressure.
  • Traditional treatments often involve hysterectomy, impacting fertility and quality of life.

Purpose of the Study:

  • To evaluate the efficacy and safety of laparoscopic leiomyoma bipolar coagulation (myolysis) for symptomatic uterine fibroids.
  • To assess symptom control, uterine volume reduction, and impact on fertility.

Main Methods:

  • 167 women with symptomatic leiomyomata underwent laparoscopic myolysis.
  • Concomitant transcervical endometrial resection (TEMR) and submucous resection (TSR) were performed for chronic menorrhagia.
  • Preoperative leuprolide acetate was administered to 98.2% of patients.

    • The procedure offers significant symptom relief and uterine volume reduction.
    • Selected women contemplating pregnancy may benefit, but further research is warranted.
