Colonic motility in proctalgia fugax

    Summary

    Proctalgia fugax pain in some patients originates from sigmoid colon contractions, not anal or rectal muscles. This suggests proctalgia fugax may be a variant of irritable bowel syndrome with referred pain.

    Background:

    • Proctalgia fugax is a poorly understood condition characterized by recurrent episodes of severe anal pain.
    • Previous theories suggested pain originated from anal sphincters, levator ani muscles, or rectal wall spasm.

    Observation:

    • Intraluminal pressure recordings were performed on the rectum and sigmoid colon in two patients during proctalgia fugax attacks.
    • Pain episodes in both patients correlated with contractions of the sigmoid colon.

    Findings:

    • The study found that sigmoid colon contractions, not anal or rectal muscle spasms, were the source of pain in these patients.
    • This challenges previous etiological explanations for proctalgia fugax.

    Implications:

    • Proctalgia fugax may represent an atypical presentation of irritable bowel syndrome (IBS) in some individuals.
    • Pain in these cases appears to be referred from the sigmoid colon to the rectal area.
    • Further research is warranted to explore the link between IBS and proctalgia fugax.
