Summary

Interferon-alpha treatment successfully reverted accelerated phase chronic myelogenous leukemia to the chronic phase. This therapy eliminated both hypotetraploid and hypooctoploid Philadelphia chromosome-positive cell clones.

Background:

  • Chronic myelogenous leukemia (CML) is a myeloproliferative neoplasm characterized by the Philadelphia chromosome.
  • Transformation to accelerated phase indicates disease progression and poorer prognosis.
  • Interferon-alpha has been used in CML treatment, but its efficacy in advanced phases with complex cytogenetics is of interest.

Observation:

  • A patient presented with chronic myelogenous leukemia in the accelerated phase.
  • Peripheral blood and bone marrow showed increased myeloblasts and promyelocytes, with elevated platelet counts and frequent megakaryocytes.
  • Cytogenetic analysis revealed hypotetraploid and hypooctoploid cell populations, each with multiple Philadelphia chromosomes.

Findings:

  • Interferon-alpha therapy led to the disappearance of the hypooctoploid clone.
