Area of Science:

  • Cardiovascular Physiology
  • Metabolic Syndrome
  • Endocrinology

Background:

  • Insulin resistance and hyperinsulinemia are increasingly linked to hypertension.
  • Vascular transport of insulin and glucose to muscles may limit their effects.
  • The study investigates the interplay between hypertension, insulin, and glucose metabolism.

Purpose of the Study:

  • To examine the impact of experimentally induced hypertension on muscle glucose uptake.
  • To determine if exogenous hyperinsulinemia influences blood pressure in normotensive and hypertensive rats.
  • To assess the role of insulin in human essential hypertension through exercise interventions.

Main Methods:

  • Assessed muscle uptake of a glucose analog ([(3)H]-deoxyglucose) in renovascular and DOCA-salt hypertensive rat models.
  • Administered sustained-release insulin implants to normotensive and spontaneously hypertensive rats (SHR).
  • Evaluated the effects of a 3-month exercise program on glucose tolerance, insulin clearance, and blood pressure in human essential hypertension.

Main Results:

  • Induced hypertension models showed altered [(3)H]-deoxyglucose uptake compared to controls.
  • Normotensive rats with exogenous hyperinsulinemia did not develop resting hypertension.
  • Young SHR with induced hyperinsulinemia exhibited accelerated hypertension development.
  • Exercise training improved glucose tolerance and insulin clearance in humans but did not lower 24-h ambulatory blood pressure.

Conclusions:

  • Hypertension does not cause insulin resistance, and exogenous hyperinsulinemia does not induce hypertension in normotensive individuals.
  • Genetic predisposition, rather than hypertension itself, is associated with insulin's effect on blood pressure.
  • In essential hypertension, insulin does not appear to significantly contribute to maintaining elevated blood pressure.
