Area of Science:

  • Vascular Surgery
  • Endovascular Interventions
  • Aortic Aneurysm Repair

Background:

  • Abdominal aortic aneurysms (AAAs) pose significant risks.
  • Conventional surgical repair has associated morbidity and mortality.
  • Endovascular aneurysm repair (EVAR) offers a less invasive alternative.

Purpose of the Study:

  • To evaluate the efficacy and safety of endovascular abdominal aortic aneurysm (AAA) exclusion.
  • To report outcomes using polyester-covered nitinol stent-grafts.
  • To assess the feasibility of endoluminal repair for infrarenal AAAs.

Main Methods:

  • A two-center study involving 149 patients undergoing EVAR for infrarenal AAAs.
  • Stent-grafts (Mialhe Stentor, Vanguard) were used in tube or bifurcated configurations.

Related Experiment Videos

  • Inclusion criteria focused on specific anatomical parameters of the proximal aorta and iliac arteries.
  • Main Results:

    • Primary technical success rate was 87% (130/149 patients).
    • 30-day mortality was 0.7%, with no late deaths during a 13.5-month average follow-up.
    • Secondary patency was 96%, with successful management of endoleaks and graft limb thromboses.

    Conclusions:

    • Endoluminal repair of infrarenal AAAs using stent-grafts is a viable alternative to open surgery.
    • Further long-term studies are needed to confirm the durability of this endovascular approach.
    • Refined endograft models may further improve outcomes in AAA treatment.
