    Background:

    • Cyclophosphamide is a widely used chemotherapy agent for various pediatric malignancies.

    Observation:

    • Two pediatric patients developed chronic respiratory failure after receiving cyclophosphamide for lymphocytic leukemia and Hodgkin disease.
    • Both patients received cumulative doses under 70g of cyclophosphamide during their initial treatment phases.

    Findings:

    • Autopsy and lung biopsy revealed severe pulmonary fibrosis in both patients.
    • Patients exhibited reduced lung compliance, thoracic deformities with decreased anteroposterior diameter, and one experienced recurrent pneumothoraces.

    Implications:

    • This highlights a potential severe pulmonary toxicity associated with cyclophosphamide in pediatric cancer survivors.
    • Further research is needed to understand the mechanisms and long-term pulmonary sequelae of cyclophosphamide treatment.
