Background:

  • Enterobacteria, particularly Klebsiella species, are suspected in the development of ankylosing spondylitis.
  • Understanding the gut microbiome's role in spondyloarthropathy is crucial for effective treatment.

Purpose of the Study:

  • To comprehensively analyze the fecal flora in patients with ankylosing spondylitis.
  • To determine if Klebsiella or other arthritogenic organisms are more prevalent in spondyloarthropathy patients compared to healthy controls.

Main Methods:

  • Faecal samples from 82 ankylosing spondylitis patients and 36 healthy controls were analyzed.
  • Microbial composition was compared between patient and control groups.
  • Correlation between Klebsiella presence and disease activity markers was assessed.

Main Results:

  • The diversity of gut flora was similar between ankylosing spondylitis patients and healthy individuals.
  • No elevated isolation rate of Klebsiella or other proposed arthritogenic bacteria was observed in patients.
  • Klebsiella presence, when identified, did not correlate with disease activity, ESR, or CRP levels.

Conclusions:

  • The study does not support the hypothesis that Klebsiella species play a significant role in the aetiopathogenesis of ankylosing spondylitis.
  • Further research is needed to fully elucidate the gut microbiome's involvement in spondyloarthropathy.
