Interferon-alpha (IFN-alpha) treatment for hepatitis C can trigger inflammatory polyarthritis. Preexisting autoantibodies and HLA-DR4 may contribute to this autoimmune side effect.

Area of Science:

  • Rheumatology
  • Hepatology
  • Immunology

Background:

  • Chronic hepatitis C infection is a common condition.
  • Interferon-alpha (IFN-alpha) has been used to treat hepatitis C.
  • Autoimmune diseases can be triggered or exacerbated by certain medications.

Observation:

  • A patient developed progressive inflammatory polyarthritis following IFN-alpha treatment.
  • The patient's arthritis worsened upon rechallenge with IFN-alpha.
  • Discontinuation of IFN-alpha did not lead to remission of the arthritis.

Findings:

  • The patient had pre-existing autoantibodies.
  • The patient was positive for HLA-DR4.
  • These factors may play a role in the development of IFN-alpha-associated autoimmune disease.

Related Experiment Videos

Implications:

  • IFN-alpha therapy can induce or unmask autoimmune conditions like polyarthritis.
  • Identifying patients with predisposing factors (autoantibodies, HLA-DR4) may help predict risk.
  • Further research is needed to understand the mechanisms of drug-induced autoimmunity.
