Epstein Barr virus (EBV) infection can manifest unusually. Three diverse cases highlight unique clinical presentations of EBV, all responding to tailored antiviral and immunomodulator treatments.

Background:

Observation:

  • Case I: A 47-year-old female with systemic lupus erythematosus presented with infectious mononucleosis, developing polyclonal hypergammaglobulinemia with IgM predominantly against EBV.
  • Case II: A 32-year-old male exhibited central neurological alterations and demyelinating encephalitis with IgM antibodies against EBV, confirmed by brain stem MRI.
  • Case III: A 40-year-old male developed a right tonsil tumor (villous leukoplakia) with IgM antibodies against EBV.

Findings:

  • All three patients demonstrated distinct clinical courses associated with Epstein Barr virus.
