Summary

For primary open-angle glaucoma, 35 argon laser trabeculoplasty burns are as effective as 50 burns for reducing intraocular pressure. This finding suggests fewer laser applications may be sufficient for effective treatment.

Background:

  • The optimal number of laser applications for argon laser trabeculoplasty (ALT) in primary open-angle glaucoma (POAG) is not well-established.
  • Determining the minimum effective number of burns can optimize treatment and potentially reduce complications.

Purpose of the Study:

  • To determine the fewest number of ALT burns required for clinically effective intraocular pressure (IOP) reduction in POAG patients.

Main Methods:

  • A prospective, randomized, collaborative study involving 122 POAG patients.
  • Patients were assigned to receive either 50 burns (half trabecular meshwork) or 35 burns (one-third trabecular meshwork).
  • Intraocular pressure was measured at baseline and 9-12 months post-procedure.

  • Baseline IOP was similar between the 50-burn and 35-burn groups (22.3 ± 4.1 mm Hg vs. 22.8 ± 5.0 mm Hg).
  • Both groups showed significant IOP reduction at 9-12 months (P < .0001).
  • No significant difference in IOP reduction was observed between the 35-burn and 50-burn groups (P = .63).

Conclusions:

  • Thirty-five burns to the trabecular meshwork may be as clinically effective as 50 burns for IOP reduction in POAG.
  • Fewer laser applications could be sufficient for effective ALT treatment in POAG patients.
