Nasal allergy and chronic sinusitis share similar symptoms and inflammatory markers. Surgery significantly improved both conditions, suggesting chronic sinusitis is a primary driver of nasal symptoms when co-occurring with allergies.

Area of Science:

  • Otolaryngology
  • Allergy and Immunology
  • Inflammatory Diseases

Background:

  • Nasal allergy is a known risk factor for inflammatory chronic sinusitis.
  • The exact immunological mechanism linking nasal allergens to sinus modifications remains unclear.
  • Shared symptoms, eosinophilic inflammation, and inflammatory mediators are observed in allergic rhinitis and sinusitis.

Purpose of the Study:

  • To investigate the role of IgE-mediated mechanisms in sinus reactions to nasal allergens.
  • To compare the efficacy of ethmoidectomy in patients with chronic ethmoidal sinusitis, with or without perennial nasal allergy.
  • To determine the primary factor responsible for nasal symptoms in patients with co-existing conditions.

Main Methods:

  • A prospective study involving 106 patients with bilateral chronic ethmoidal sinusitis.

Related Experiment Videos

  • 24 patients were allergic to perennial allergens and received 3 months of antiallergic treatment before surgery.
  • Bilateral endonasal ethmoidectomy was performed, and symptom scores (rhinorrhea, nasal obstruction, global comfort) were compared pre- and post-surgery.
  • Main Results:

    • Both allergic and non-allergic groups showed significant improvement in nasal symptoms after ethmoidectomy.
    • No significant difference in symptom improvement was observed between the two groups post-surgery.
    • This indicates shared symptomology and comparable surgical outcomes regardless of allergic status.

    Conclusions:

    • Symptoms of perennial nasal allergy and chronic ethmoidal sinusitis are often indistinguishable.
    • Failure to respond to allergy treatment warrants sinus imaging to rule out concurrent chronic sinusitis.
    • Chronic ethmoidal sinusitis appears to be the predominant factor causing nasal obstruction and rhinorrhea in patients with co-existing perennial allergy.
