Laparoscopic cholecystojejunostomy using an endoscopic stapler offers a minimally invasive palliative option for biliary obstruction in pancreatic cancer patients. This technique provides rapid recovery and clinical improvement compared to open surgery.

Background:

  • Biliary obstruction and jaundice are common complications of inoperable pancreatic carcinoma.
  • Patients with advanced pancreatic cancer often have comorbidities making them poor candidates for open surgery.

Observation:

  • Laparoscopic cholecystojejunostomy was performed using an endoscopic gastrointestinal stapler in two patients with unresectable pancreatic cancer.
  • The procedure was completed in 45-60 minutes, excluding non-operative delays.

Findings:

  • Both patients experienced immediate and significant clinical improvement.
  • Minimal operative sequelae were observed, with one patient discharged in 4 days and the other in less than 24 hours.

Implications:

  • Laparoscopic cholecystojejunostomy is a viable and effective palliative surgical technique for managing biliary obstruction in select pancreatic cancer patients.
