Area of Science:

  • Dermatopathology
  • Surgical Oncology
  • Oncologic Imaging

Background:

  • Eccrine spiradenoma is a benign adnexal neoplasm.
  • Malignant transformation is rare, with few reported cases.
  • Perineal involvement is an unusual presentation.

Observation:

  • A 60-year-old male presented with a perineal mass.
  • Histopathological examination revealed areas of typical eccrine spiradenoma admixed with carcinomatous and sarcomatous components.
  • Immunohistochemical and ultrastructural analyses showed no epithelial differentiation in the sarcomatous areas.

Findings:

  • The tumor was classified as a carcinosarcoma arising in eccrine spiradenoma.
  • The patient experienced an aggressive clinical course.

Related Experiment Videos

  • Rapid development of nodal and pulmonary metastases was observed.
  • Implications:

    • This case highlights the aggressive potential of malignant eccrine spiradenoma.
    • Accurate diagnosis through immunohistochemistry and ultrastructural analysis is crucial.
    • Understanding the behavior of carcinosarcoma arising in eccrine spiradenoma is vital for patient management.
