Summary

Severe inflammation can lower serum cortisol levels, potentially worsening illness. Physiologic hydrocortisone doses may help manage these effects and improve outcomes in patients with critical illness.

Area of Science:

  • Endocrinology
  • Critical Care Medicine
  • Inflammation Research

Background:

  • Severe inflammation is a complex physiological state impacting multiple organ systems.
  • The adrenal response to critical illness, including cortisol production, is not fully understood.
  • Assessing cortisol levels is crucial for managing critically ill patients.

Observation:

  • This study observed cortisol levels in six patients experiencing severe inflammation.
  • Measurements included basal and cosyntropin-stimulated serum cortisol.
  • Cortisol levels were assessed during both severe and less severe inflammatory periods.

Findings:

  • Patients exhibited lower-than-expected serum cortisol levels during severe inflammation.
  • Cortisol levels significantly increased as inflammation subsided (P=.03).

Related Experiment Videos

  • Five patients received physiologic hydrocortisone doses, leading to decreased inflammatory manifestations.
  • Implications:

    • Severe inflammation may induce transient hypoadrenalism, exacerbating critical illness.
    • Physiologic hydrocortisone replacement may be beneficial in managing severe inflammation.
    • Understanding cortisol dynamics is key to optimizing treatment strategies in critical care.
