Background:

Lumbosacral defects present significant reconstructive challenges.
Traditional methods often involve substantial donor site morbidity.

Observation:

A perforator-based flap technique was utilized for 20 patients with lumbosacral defects.
Vascular pedicles were sourced from intercostal, lumbar, and gluteal perforators.
The gluteus maximus muscle was preserved in this approach.

Findings:

The study reported minor complications in five cases.
This method minimizes donor site morbidity by avoiding gluteus maximus sacrifice.
The technique proved effective for both ambulatory and paraplegic patients.
