This study details a rare ectopic prolactinoma found in the clivus, unrelated to the pituitary gland. This finding is significant as only one other similar case of ectopic pituitary adenoma has been documented.

Background:

  • Ectopic pituitary adenomas are exceptionally rare, with limited documented cases.
  • While the clivus can be involved, ectopic adenomas are typically unrelated to the sella turcica.

Observation:

  • A 47-year-old woman presented with headache, and imaging revealed a clival tumor.
  • Radiological assessment showed no connection to the pituitary gland, leading to a preoperative diagnosis of metastatic cancer.

Findings:

  • Surgical removal via a transsphenoidal approach yielded a hemorrhagic tumor.
  • Neuropathological examination confirmed the tumor as a prolactinoma, distinct from the normal pituitary gland.

Implications:

  • This case represents a rare instance of an ectopic prolactinoma in the clivus.
