A rare malignant nodular hidradenoma recurred multiple times over 30 years in a farmer. This aggressive skin cancer showed local recurrence and lymph node metastasis despite wide excision.

Area of Science:

  • Dermatopathology
  • Surgical Oncology

Background:

  • Nodular hidradenoma, a rare skin adnexal neoplasm, can exhibit malignant potential.
  • This case highlights the diagnostic challenges and aggressive behavior of malignant nodular hidradenoma.

Observation:

  • A 54-year-old male farmer presented with a 30-year history of a slow-growing, exulcerated chest tumor.
  • Initial histopathology revealed an epithelial neoformation consistent with malignant nodular hidradenoma, characterized by clear polygonal cells and basaloid cells with atypia, set in myxoid stroma.

Findings:

  • Despite wide excision, the tumor recurred locally within 2 years and again one year after a second excision.
  • Metastasis to an axillary lymph node was confirmed after the third recurrence, indicating aggressive disease progression.

Related Experiment Videos

Implications:

  • Malignant nodular hidradenoma requires vigilant follow-up due to its high recurrence rate and metastatic potential.
  • Early recognition and aggressive management are crucial for improving outcomes in patients with this rare malignancy.
