Summary

Routine supplemental breast ultrasonography (US) for dense breasts did not significantly improve cancer detection accuracy. While US identified abnormalities, many were benign cysts or nodules, and some cancers were missed or overdiagnosed.

Background:

  • Dense breast tissue can obscure abnormalities on mammograms.
  • Ultrasonography is often used as a supplemental screening tool.

Purpose of the Study:

  • To evaluate the effectiveness of routine supplemental ultrasonography in screening asymptomatic patients with dense breasts.

Main Methods:

  • 350 asymptomatic patients with dense breasts underwent bilateral ultrasonography using 7.5, 10, and 13 MHz transducers.
  • Findings were compared with mammograms and clinical follow-up.

Main Results:

  • 165/350 patients had abnormalities: 117 cysts, 44 solid nodules, 4 mixed nodules. Two invasive carcinomas were detected by US but not mammography.

  • Mammography retrospectively identified 24.7% of cysts and 31% of solid nodules.
  • Four false positive diagnoses of breast cancer occurred for fibroadenomas.

    • Routine supplemental ultrasonography for dense breasts does not significantly enhance the accuracy of breast cancer work-up.
    • Mammography retrospectively identified a substantial portion of lesions seen on ultrasound.
