Summary

Pregnancy outcomes for women on dialysis have significantly improved, with higher success rates when dialysis begins before or during pregnancy. Increased dialysis doses and erythropoietin (EPO) are key to successful pregnancies in end-stage renal failure patients.

Area of Science:

  • Nephrology
  • Obstetrics
  • Reproductive Medicine

Background:

  • Pregnancy in end-stage renal failure patients on dialysis historically had poor outcomes.
  • Limited literature exists on pregnancy experiences of patients undergoing dialysis.
  • Erythropoietin (EPO) use and dialysis parameters are crucial factors.

Purpose of the Study:

  • To review outcomes of pregnancies in dialysis patients.
  • To analyze the impact of dialysis technique, dose, and EPO on pregnancy.
  • To assess blood pressure evolution and obstetric data in this population.

Main Methods:

  • Retrospective review of five pregnancies in dialysis patients.
  • National survey of obstetric data from dialysis patients.
  • Analysis of dialysis frequency, dose (weekly Kt/V), and EPO administration.

Related Experiment Videos

Main Results:

  • Successful pregnancy rates were 50% for hemodialyzed patients and 80% for those starting dialysis after conception.
  • Increased dialysis dose (weekly Kt/V of 6-8) and EPO were necessary; no EPO-related hypertension occurred.
  • All live births resulted in low birth weight and prematurity; polyhydramnios and growth retardation were observed.

Conclusions:

  • Pregnancy prognosis for dialysis patients has markedly improved.
  • Higher dialysis doses correlate with better birth weights in successful pregnancies.
  • Careful management of dialysis and EPO can optimize pregnancy outcomes.
