Subtotal petrosectomy enables cochlear implantation in patients with complex ear conditions previously deemed unsuitable. This surgical technique safely expands cochlear implant candidacy for challenging cases.

  • Cochlear implantation is often contraindicated in patients with abnormal middle/inner ear conditions, intracranial communication, or recurrent infections.
  • These conditions pose risks during foreign body insertion, limiting candidacy for advanced hearing restoration.

Observation:

  • Five patients with complex ear pathologies (Mondini dysplasia with CSF leak, atretic mastoid with meningocele, chronic otitis media, petrous bone fracture, temporal bone adenoma) were evaluated.
  • All patients underwent subtotal petrosectomy combined with cochlear implantation, with four cases done simultaneously and one staged procedure.

Findings:

  • The subtotal petrosectomy technique successfully isolated and obliterated the tympanomastoid cleft in all cases.
  • No intraoperative or postoperative complications were reported following the combined procedures.
