Summary
Proton pump inhibitor users with persistent gastritis symptoms may have gastric giardiasis. Further investigation for this parasite is recommended, especially if Helicobacter pylori or intestinal metaplasia is present.
Area of Science:
- Gastroenterology
- Infectious Diseases
- Parasitology
Background:
- Proton pump inhibitors (PPIs) are commonly used for acid-related gastrointestinal disorders.
- Persistent symptoms of gastritis or peptic ulcers despite PPI treatment warrant further investigation.
- Gastric giardiasis is a less common but potential cause of upper gastrointestinal symptoms.
Purpose of the Study:
- To investigate the prevalence of gastric giardiasis in patients with persistent gastritis or peptic ulcer symptoms on PPI therapy.
- To explore potential associations between gastric giardiasis, H. pylori, and intestinal metaplasia.
Main Methods:
- Upper gastrointestinal endoscopy was performed on 30 patients.
- Gastric biopsies were obtained for microscopic examination to detect giardiasis.
- Histopathological analysis included assessment for H. pylori and intestinal metaplasia.
Main Results:
- Gastric giardiasis was detected in 3 out of 30 patients (10% prevalence).
- All cases of gastric giardiasis were associated with the presence of H. pylori and/or intestinal metaplasia.
- No significant difference in symptoms was noted between patients with and without giardiasis.
Conclusions:
- A potential relationship exists between gastric giardiasis and proton pump inhibitor use.
- Endoscopists should consider screening for gastric giardiasis in patients with persistent symptoms, particularly those with H. pylori or intestinal metaplasia.
