Summary

Chloroquine toxicity can cause severe cardiac and muscle damage in patients with rheumatoid arthritis. Early endomyocardial biopsy is crucial for diagnosing chloroquine-induced heart dysfunction.

Observation:

  • A 60-year-old woman with rheumatoid arthritis on long-term chloroquine developed heart block, heart failure, and myopathy.
  • She died of pulmonary thromboembolism during myopathy investigations.

Findings:

  • Cardiac and skeletal muscle changes consistent with chloroquine toxicity were found.
  • Hypertension and amyloidosis were considered but chloroquine toxicity was observed.

Implications:

  • Chloroquine may be an unrecognized contributor to cardiac dysfunction in rheumatological patients.
  • Early endomyocardial biopsy is recommended for diagnosing potential chloroquine-induced cardiac issues.
