Primary hyperparathyroidism (PHP) and amyotrophic lateral sclerosis (ALS) may coexist. Surgical removal of parathyroid adenomas in ALS patients did not change the disease progression, suggesting these conditions are unrelated.

Area of Science:

  • Neurology
  • Endocrinology
  • Oncology

Background:

  • An association between primary hyperparathyroidism (PHP) and amyotrophic lateral sclerosis (ALS) has been observed.
  • The causal relationship between PHP and ALS remains unconfirmed.

Observation:

  • Five patients meeting El Escorial criteria for ALS also had PHP.
  • PHP diagnosis timing varied, preceding ALS onset by up to two years in one case.
  • All patients exhibited elevated serum calcium and parathyroid hormone (PTH) levels.

Findings:

  • Parathyroid adenoma resection normalized serum calcium and PTH levels in all patients.
  • Despite normalized levels, all patients experienced progressive weakness and died within 1-3 years post-surgery.
  • Surgical intervention did not alter the clinical course or prognosis of ALS.

Related Experiment Videos

Implications:

  • PHP and ALS appear to be distinct, coexisting disorders.
  • Treatment of PHP does not impact the progression of ALS.
  • Further research is needed to understand the underlying mechanisms of ALS and potential co-occurrence with endocrine disorders.
