    Background:

    • Endoscopic retrograde cholangiopancreatography (ERCP) is a common procedure for biliary and pancreatic diseases.
    • Papillotomy, a component of ERCP, involves incising the papillary sphincter to facilitate stone removal or relieve obstruction.
    • High-risk patients often face contraindications for traditional surgical interventions.

    Purpose of the Study:

    • To evaluate the efficacy and safety of endoscopic papillotomy using an experimental insulated duodenoscope.
    • To assess the procedure's applicability in patients with common bile duct stones and papillary stenosis who are poor surgical candidates.

    Main Methods:

    • Endoscopic papillotomy was performed on 10 patients under diazepam sedation using an experimental insulated duodenoscope and diathermy wire.
    • Patients included 7 with common bile duct stones (6 post-cholecystectomy) and 3 with papillary stenosis.
    • Nine patients had significant medical or surgical contraindications for operative treatment.
