Summary

Elevated liver enzymes are common in myotonic dystrophy (DM) patients, even without apparent liver damage. This finding suggests liver enzyme abnormalities should be recognized as a potential laboratory marker for DM.

Background:

  • Myotonic dystrophy (DM) is a multisystem disorder with limited evidence of liver involvement.
  • Previous studies have not extensively investigated liver and gallbladder function in DM patients.

Purpose of the Study:

  • To prospectively assess liver and gallbladder function in 53 myotonic dystrophy patients.
  • To correlate liver function test results with clinical and genetic parameters of DM.

Main Methods:

  • Prospective evaluation of 53 DM patients.
  • Assay of six liver enzymes (alkaline phosphatase, gamma-glutamyltransferase, 5' nucleotidase, aspartate aminotransferase, alanine aminotransferase, lactate dehydrogenase).
  • Correlation analysis with clinical data, creatine kinase, glucose, lipids, and genetic testing (CT-G repeat expansion).

  • 66% of patients showed abnormal serum activity in at least one liver enzyme.
  • Elevated alkaline phosphatase (50.9%) and gamma-glutamyltransferase (52.8%) were most common.
  • No correlation found between liver enzyme abnormalities and disease severity, duration, or genetic factors.

Conclusions:

  • Elevated liver enzymes are a frequent laboratory finding in myotonic dystrophy.
  • Liver enzyme abnormalities should be considered an additional characteristic of DM.
  • Further research may explore the underlying mechanisms of liver enzyme elevation in DM.
