Summary

Spinal epidural empyema, a rare bacterial abscess in the epidural space, caused severe spinal cord compression in two dogs. This condition, previously unreported in veterinary literature, presents a poor prognosis.

Background:

  • Myelography revealed extensive, diffuse epidural spinal cord compression in both dogs.
  • Purulent fluid with bacteria was aspirated from the epidural space under fluoroscopic guidance.

Findings:

  • One dog showed poor response to aggressive medical management, including epidural lavage and drainage.
  • Both dogs were euthanized due to disease severity and poor prognosis.
  • Bacterial infection within the epidural space led to severe spinal cord compression.

Implications:

  • Spinal epidural empyema should be considered in canine myelopathies, particularly those with fever.
  • Early recognition and diagnosis are crucial for potential treatment of this rare condition.
  • This study highlights a novel differential diagnosis for acute neurological deficits in dogs.
