Insights
Dobutamine stress echocardiography effectively detects coronary artery disease in patients with chest pain and abnormal ECGs. However, its low negative predictive value means further testing may be needed.
Area of Science:
- Cardiology
- Diagnostic Imaging
Background:
- Coronary artery disease (CAD) is a leading cause of chest pain.
- Electrocardiogram (ECG) abnormalities can indicate underlying cardiac issues.
- Accurate diagnostic tools are crucial for timely CAD detection.
Purpose of the Study:
- To evaluate the effectiveness of dobutamine stress echocardiography (DSE) in diagnosing CAD.
- To compare DSE with exercise ECG in patients presenting with chest pain and abnormal resting ECGs.
Main Methods:
- Dobutamine stress echocardiography was performed on 218 patients.
- Coronary angiography was used to define significant CAD (>50% stenosis).
- A subgroup of 69 patients underwent treadmill exercise testing.
Main Results:
- DSE demonstrated a sensitivity of 89% and specificity of 50% for CAD detection.
- Sensitivity for multi-vessel, double-vessel, and single-vessel disease was high (97%, 82%, 81%).
- Treadmill exercise testing showed a lower sensitivity (37%) in a subgroup.
Conclusions:
- Dobutamine stress echocardiography is superior to exercise ECG for identifying significant CAD.
- The negative predictive value of DSE in this patient cohort is limited, necessitating careful interpretation.
Purpose:
We assessed the value of dobutamine stress echocardiography for the detection of coronary artery disease in patients with chest pain and an abnormal resting electrocardiograph (ECG).
Methods:
Dobutamine stress echocardiography was performed in a standard fashion. Significant coronary artery disease was defined as a >50% luminal diameter stenosis on coronary angiography.
Results:
The sensitivity, specificity, positive and negative predictive value of dobutamine stress echocardiography for the detection of coronary artery disease in 218 patients were 89, 50, 95 and 32%, respectively. The sensitivity for detection of multi-, double- and single vessel disease were 97, 82 and 81%, respectively. The sensitivity for the detection of coronary artery disease in a subgroup of 69 patients by treadmill exercise testing was 37%.
Conclusion:
Dobutamine stress echocardiography is better than exercise ECG for the detection of significant coronary artery disease. The negative predictive value of dobutamine stress echocardiography in this patient group is low.

Exercise Stress Test
Exercise stress testing, commonly known as a treadmill test, is a noninvasive procedure used to evaluate cardiovascular function and diagnose heart conditions.
Definition
An exercise stress test measures the heart's response to exertion using a treadmill or stationary bicycle. Chest electrodes record the heart's electrical activity through an ECG, and blood pressure is monitored regularly.
