Summary

Screening for tuberculosis disease is not efficient for routine practice. However, screening to prevent tuberculosis disease is effective when combined with preventive chemotherapy, identifying at-risk individuals.

Area of Science:

  • Public Health
  • Infectious Disease Epidemiology

Background:

  • Screening is a crucial health activity with established guidelines.
  • Tuberculosis (TB) screening methods exist and have been assessed.
  • Current screening criteria are largely applicable to TB.

Purpose of the Study:

  • To evaluate the efficiency and recommendations for tuberculosis screening.
  • To determine the utility of screening for tuberculosis disease prevention.
  • To assess the ethical considerations of screening and preventive chemotherapy.

Main Methods:

  • Review of existing tuberculosis screening methodologies.
  • Analysis of screening efficiency for disease versus prevention.
  • Consideration of diagnostic tools: tuberculin skin test, chest radiograph, Mycobacterium tuberculosis cultures.

Main Results:

  • Screening for active tuberculosis disease is inefficient and not routinely recommended.
  • Screening for tuberculosis disease prevention is efficient when preventive chemotherapy is integrated.
  • Identification of high-risk populations for targeted preventive chemotherapy is feasible.

Conclusions:

  • Screening for tuberculosis disease should not be a routine practice.
  • Screening for tuberculosis prevention is indicated in high-probability groups, necessitating preventive chemotherapy.
  • Preventive chemotherapy guidelines must weigh potential harms against benefits; screening without it is unethical.
