Summary

Patients with head and neck cancer and severe carotid stenosis can undergo simultaneous carotid endarterectomy and neck dissection. This combined procedure shows promising results with no increased morbidity for treating metastatic neck disease.

Background:

  • Patients with upper aerodigestive tract carcinomas often have a history of tobacco and alcohol abuse, predisposing them to atherosclerotic vascular disease.
  • Head and neck cancers and their radiotherapy treatments are associated with an increased incidence and severity of carotid stenosis.

Observation:

  • Management of severe carotid stenosis concurrently with surgical treatment for metastatic neck disease in cancer patients was not previously described.
  • A prospective study was conducted involving three patients with severe bilateral carotid stenosis (>90%) requiring surgery for metastatic neck disease.

Findings:

  • All three patients underwent simultaneous carotid endarterectomy with saphenous vein grafting and modified radical neck dissections.

  • Two patients experienced successful healing with no local morbidity, no neck recurrence, and patent carotid arteries.
  • No strokes occurred in the study cohort; one patient died of myocardial infarction.
