Summary

Scleral resections effectively treated exudative retinal detachment in nanophthalmos. Covering sclerectomized areas with gelatine film may prevent recurrence of this vision-threatening condition.

Background:

  • Nanophthalmos is a rare congenital condition characterized by a small, hyperopic eye.
  • Exudative retinal detachment can occur secondary to nanophthalmos, often associated with angle-closure glaucoma.
  • Management of exudative retinal detachment in nanophthalmos presents unique surgical challenges.

Observation:

  • A 46-year-old male with bilateral nanophthalmos developed acute angle-closure glaucoma in his left eye.
  • Following a peripheral iridectomy, the patient presented with exudative retinal detachment.
  • The exudative retinal detachment persisted, necessitating further surgical intervention.

Findings:

  • A surgical procedure involving partial thickness sclerectomy with central sclerostomy, covered by absorbable gelatine film, was performed on the affected eye.
  • Gradual resolution of the retinal detachment was observed post-operatively.
  • Complete retinal reattachment was achieved within eight months.
  • Visual acuity improved significantly from hand movements to 6/18 with correction.

Implications:

  • Scleral resections represent a viable and effective treatment for nanophthalmic retinal detachment.
  • The use of absorbable gelatine film appears to be beneficial in preventing scleral tissue regeneration and potential recurrence.
  • This surgical approach offers a promising strategy for visual recovery in patients with nanophthalmos and associated retinal detachment.
