Background:

  • Cat-scratch disease (CSD) is a bacterial infection caused by Bartonella henselae.
  • Neurologic manifestations of CSD are uncommon, typically presenting as encephalopathy or meningitis.

Observation:

  • A 10-year-old girl presented with an enhancing, gallium-avid mass in the left middle cranial fossa.
  • Imaging findings included CT, MR imaging of the brain, and gallium-67 scintigraphy.

Findings:

  • Craniotomy revealed an inflammatory mass involving the left trigeminal nerve.
  • Histopathology identified rodlike bacteria within the lesion.
  • Serologic tests confirmed positivity for cat-scratch disease.

Implications:

  • This case represents an unusual presentation of neurologic involvement in cat-scratch disease.
  • The detection of Bartonella henselae organisms within neural tissue is a novel finding.
  • Highlights the importance of considering CSD in the differential diagnosis of cranial masses, even with atypical presentations.
