Video-assisted thoracoscopic surgery (VATS) is a safe and effective procedure for diagnosing and treating various thoracic conditions. VATS demonstrated low complication rates and high success rates for conditions like pleural effusion and lung nodules.

Background:

Purpose of the Study:

  • To review the experience with 341 cases of video-assisted thoracoscopic surgery (VATS).
  • To analyze the indications, operative procedures, and complication or failure rates associated with VATS.
  • To evaluate the efficacy of VATS in various thoracic conditions.

Main Methods:

  • A retrospective review of 459 video-assisted thoracoscopic procedures performed over 3 years.
  • Analysis included patient demographics (206 male, 135 female) and procedure indications (171 diagnostic, 170 therapeutic).

  • Data collected encompassed operative procedures, complications, conversion rates, and diagnostic/therapeutic success rates.
  • Main Results:

    • No operative mortality was observed; non-fatal complications occurred in 4.4% of cases.
    • Mean postoperative hospital stay was 5 days.
    • High diagnostic yields were achieved for pulmonary nodules/masses (100%) and undiagnosed pleural effusions (95.2%). The success rate for non-tuberculous thoracic empyema was 87.3%.

    Conclusions:

    • Video-assisted thoracoscopic surgery is an ideal approach for undiagnosed pleural effusion, recurrent pneumothorax, and bullous lung disease.
    • VATS is also highly effective for stage II thoracic empyema, lung cancer staging, peripheral pulmonary nodules, and wedge biopsies for diffuse lung disease.
