Summary

Iodinated contrast media can trigger hyperthyroidism in elderly women with subclinical goiters. Though rare, this iodine-induced hyperthyroidism often resolves spontaneously, even without medication.

Background:

  • Subclinical hyperthyroid goiter presents a risk factor for developing thyroid dysfunction.
  • Iodinated contrast media are widely used in medical imaging.

Observation:

  • Three elderly women (63, 72, 75 years) with subclinical goiters developed hyperthyroidism after receiving iodinated contrast media.
  • Marked increases in free thyroxine (fT4) levels were observed.

Findings:

  • Two patients did not respond to thyreostatic agents; one received no treatment.
  • All three patients experienced spontaneous improvement of hyperthyroidism.
  • Iodine-induced hyperthyroidism is uncommon, typically occurring in individuals with pre-existing autonomous thyroid function.

Implications:

  • Prophylaxis with sodium perchlorate may benefit cardiac patients with goiters and low thyroid-stimulating hormone (TSH) levels.
