A rare case of endobronchial nocardiasis caused upper right lobe atelectasis, unresponsive to antibiotics. Diagnosis required multiple procedures, revealing brain involvement and bronchial mucosal calcification.

Area of Science:

  • Pulmonology
  • Infectious Diseases
  • Neurology

Background:

  • Atelectasis, particularly of the upper right lobe, can present with varied etiologies.
  • Antibiotic therapy is a standard initial approach for suspected infectious causes of lung consolidation.

Observation:

  • A patient presented with upper right lobe atelectasis refractory to antibiotic treatment.
  • Repeated bronchoscopy and bronchoalveolar lavage were necessary for diagnosis.

Findings:

  • The diagnosis of endobronchial nocardiasis was established.
  • Further investigation identified a brain metastasis.
  • Immune deficiency was not evident, though bronchial mucosa calcification was noted at the site of infection.

Implications:

Related Experiment Videos

  • Endobronchial nocardiasis should be considered in refractory atelectasis cases.
  • Disseminated nocardiosis can involve the central nervous system.
  • Bronchial mucosal calcification may be a sequela of nocardial infection.
