[Case of Hakim's syndrome]

    Summary

    Hakim's syndrome, a rare neurological condition, can cause parkinsonian symptoms. Treatment with a Pudenz-type valve led to significant improvement, confirming the diagnosis in a 53-year-old man.

    Background:

    • Hakim's syndrome, also known as normal pressure hydrocephalus (NPH), is characterized by the triad of gait disturbance, urinary incontinence, and cognitive impairment.
    • Parkinsonian features can sometimes dominate the clinical presentation of NPH, posing diagnostic challenges.

    Observation:

    • A 53-year-old male presented with symptoms predominantly mimicking parkinsonian syndrome.
    • The patient's neurological examination revealed signs consistent with parkinsonism.

    Findings:

    • Implantation of a Pudenz-type valve resulted in substantial amelioration of the patient's parkinsonian manifestations.
    • The positive response to cerebrospinal fluid (CSF) shunting supports the diagnosis of Hakim's syndrome.

    Implications:

    • This case highlights the importance of considering Hakim's syndrome in the differential diagnosis of parkinsonism.
    • Cerebrospinal fluid (CSF) shunting can be an effective treatment for Hakim's syndrome presenting with parkinsonian features.
    • Early diagnosis and intervention with shunting can significantly improve outcomes for patients with Hakim's syndrome.
