Background:

  • Poikilothermia syndrome is a rare condition characterized by intrinsic thermoregulatory failure.

Observation:

  • A patient with clinical evidence of poikilothermia syndrome and long-standing multiple sclerosis was studied.
  • Neuroimaging (CT and MRI) did not reveal a hypothalamic lesion.
  • The patient's thermoregulatory responses (sweating, vasoconstriction, shivering) were systematically evaluated.

Findings:

  • Core-temperature thresholds for thermoregulatory defenses were significantly altered.
  • The sweating threshold was elevated (38.3°C vs. normal 37.0°C).
  • The vasoconstriction threshold was lowered (34.4°C vs. normal 36.4°C), and the shivering threshold was also reduced (31.8°C vs. normal 35.6°C).
  • The interthreshold ranges for sweating-vasoconstriction and vasoconstriction-shivering were substantially widened compared to normal values, resembling patterns seen after general anesthesia.

Implications:

  • This case quantifies the profound thermoregulatory dysfunction in poikilothermia syndrome.
  • The findings suggest that poikilothermia syndrome can occur independently of identifiable hypothalamic lesions.
  • The observed thermoregulatory pattern may offer insights into neurological conditions affecting temperature homeostasis.
