Multiple focal nodular hyperplasia (FNH) lesions, a rare liver condition, were found in a patient with atypical imaging features. Oral contraceptive use was discontinued, but lesions and inflammation persisted.

Area of Science:

  • Hepatology
  • Radiology
  • Oncology

Background:

  • Focal nodular hyperplasia (FNH) is a common benign liver lesion, typically presenting as a solitary or paired finding.
  • Oral contraceptive use is a known risk factor for FNH development.
  • Multiple FNH lesions are exceptionally rare and often associated with other conditions.

Observation:

  • A patient with a history of biliary cyst resection and long-term oral contraceptive use presented with over 30 FNH lesions.
  • Imaging revealed atypical characteristics, including eccentric or absent central scars and suspected fat content in several lesions.
  • An unexplained inflammatory syndrome was also noted in the patient.

Findings:

  • Biopsy confirmed the presence of fat within some FNH lesions.

Related Experiment Videos

  • Discontinuation of oral contraceptives did not lead to regression of the FNH lesions or resolution of the inflammatory syndrome.
  • Two-year follow-up indicated stable FNH lesions and persistent inflammation.
  • Implications:

    • This case highlights the rare occurrence of multiple FNH lesions with atypical imaging features.
    • The findings suggest that oral contraceptives may play a role in the development of numerous FNH lesions.
    • Further research is warranted to understand the pathogenesis and long-term implications of multiple FNH, especially in the context of associated inflammatory conditions.
