    Summary

Local application of prostaglandin E2 (PGE2) vaginal tablets effectively induced labour in breech presentations. This method achieved high rates of successful vaginal delivery without adverse events in a small case series.

    Area of Science:

    • Obstetrics and Gynecology
    • Maternal-Fetal Medicine
    • Reproductive Medicine

    Context:

    • Induction of labour in breech presentation remains a debated topic in obstetrics.
    • Previous methods for labour induction in breech presentations have varying success rates and risks.
    • The safety and efficacy of local prostaglandin E2 (PGE2) application for this indication require further investigation.

    Purpose:

    • To evaluate the effect of local application of prostaglandin E2 (Prostin E2--3 mg) vaginal tablets for induction of labour in women with breech presentation.
    • To assess the onset of labour, duration of delivery, and mode of delivery following PGE2 induction in breech cases.
    • To determine the incidence of adverse events, such as hyperstimulation, and the need for oxytocin augmentation.

    Summary:

    • Fifteen pregnancies with breech presentation were induced using local PGE2 vaginal tablets.
    • Labour commenced within 3-13 hours, with delivery durations ranging from 4-24 hours.
    • Effective labour was achieved in 93.33% of cases, with 100% successful vaginal deliveries using classical or Bracht manoeuvres; no hyperstimulation occurred.

    Impact:

    • Local PGE2 induction appears to be a viable option for initiating labour in breech presentations when pelvic scores are favourable and cephalopelvic disproportion or fetal distress are absent.
    • This study suggests that PGE2 can be safely used for labour induction in breech presentations, contributing to successful vaginal deliveries.
    • Findings support the consideration of local PGE2 induction as a potential management strategy in selected breech presentation cases, warranting further research with larger cohorts.
