Endoscopy·2012

Area of Science:

  • Gastroenterology
  • Hepatology
  • Endoscopy

Background:

  • The efficacy of repeat endoscopic retrograde cholangiopancreatography (ERCP) by the same endoscopist after an initial failed attempt is not well-established.
  • Tertiary referral centers are often suggested for repeat ERCP after unsuccessful initial procedures.

Purpose of the Study:

  • To determine the success rate of repeat ERCP performed by the same endoscopist in a subsequent session.
  • To evaluate patient outcomes following a failed index ERCP procedure.

Main Methods:

  • A retrospective review of 500 consecutive ERCP procedures at a teaching institution.
  • Analysis of repeat ERCP success rates and outcomes in patients with initial failed cannulation.

Main Results:

Related Experiment Videos

  • The initial ERCP success rate for duct cannulation was 90.8%.
  • Repeat ERCP was performed in 51% of unsuccessful cases, achieving an 87.5% success rate.
  • Needle knife use facilitated cannulation in 80% of instances, with no complications reported for repeat ERCP.

Conclusions:

  • Repeat ERCP by the same endoscopist demonstrates a high success rate of 87.5%.
  • The overall success rate, including repeat procedures, reaches 95%.
  • Repeat ERCP is an effective strategy for patients with initially unsuccessful procedures.
