An unusual case of Crohn's disease in a West Indian child
Insights
Extensive perineal ulceration in children, a rare Crohn's disease manifestation, can be challenging to diagnose. Early recognition and treatment are crucial for managing this condition and preventing complications.
Area of Science:
- Pediatric Gastroenterology
- Inflammatory Bowel Disease Research
- Dermatology
Background:
- Crohn's disease commonly presents with perianal manifestations.
- Extensive perineal ulceration is an uncommon presentation, especially in pediatric patients.
Observation:
- A case of an 11-year-old child with progressive vulvar hypertrophy and groin ulceration is detailed.
- Diagnosis of Crohn's disease was delayed by 21 months despite presenting symptoms.
Findings:
- Treatment with corticosteroids and azathioprine achieved remission.
- Steroid dosage reduction proved challenging in this case.
Implications:
- Highlights the importance of considering Crohn's disease in children with atypical perineal symptoms.
- Emphasizes the potential diagnostic delays and treatment complexities in pediatric inflammatory bowel disease.
- Suggests the need for further research into managing severe perineal Crohn's disease in children.
Abstract:
Although perianal ulceration is common in Crohn's disease, extensive perineal ulceration particularly in children is rare. The case history is presented of a child who developed progressive hypertrophy of the vulva and ulceration of the groins at the age of 11 years but diagnosis of Crohn' s disease was delayed for 21 months. Treatment with steroids and azathioprine has induced a successful remission but difficulty has been experienced in reducing the steroid dosage.

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by transmural...
Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy
Diagnostic studies
A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the colonic...
