    Area of Science:

    Background:

    • Gallbladder disease is common, but eosinophilic infiltration of the gallbladder wall is rare.
    • Peripheral eosinophilia can indicate various conditions, including parasitic infections and allergic reactions.

    Observation:

    • A 40-year-old obese female presented with symptoms suggestive of gallbladder disease and significant peripheral eosinophilia (14%).
    • Oral cholecystogram failed to visualize the gallbladder, leading to cholecystectomy.
    • Pathology revealed a cystic duct calculus and a transmural eosinophilic infiltrate of the gallbladder wall.

    Findings:

    • Post-surgery, peripheral eosinophilia normalized.
    • One year later, the patient experienced recurrent abdominal pain, diarrhea, and peripheral eosinophilia.
    • Small bowel biopsies showed focal mucosal eosinophilia during this recurrence.

    Implications:

    • Eosinophilic cholecystitis might be a distinct entity or a manifestation of eosinophilic gastroenteritis affecting the biliary tract.
    • Further research is needed to understand the etiology and management of eosinophilic cholecystitis.
