Insights

This study reports a rare case of CD5-positive follicular lymphoma (FL) in a 59-year-old male. The patient achieved complete remission after treatment with rituximab and CHOP chemotherapy.

Area of Science:

  • Hematology
  • Oncology
  • Immunophenotyping

Background:

  • Follicular lymphoma (FL) is a common type of non-Hodgkin lymphoma.
  • CD5 expression is typically absent in FL, making CD5-positive FL a rare variant.
  • Accurate diagnosis relies on detailed immunophenotypic analysis.

Observation:

  • A 59-year-old male presented with an enlarged inguinal lymph node.
  • Biopsy revealed follicular lymphoma, grade 3A.
  • Immunophenotyping confirmed CD5 and CD20 double-positivity in lymphoma cells.

Findings:

  • The patient was diagnosed with a rare CD5-positive follicular lymphoma.
  • Differential diagnoses including mantle cell lymphoma and CD5-positive diffuse large B-cell lymphoma were excluded.
  • Treatment with rituximab and CHOP chemotherapy resulted in complete remission.

Implications:

  • This case highlights the importance of immunophenotyping in diagnosing rare lymphoma subtypes.
  • CD5-positive FL may have distinct clinical characteristics and incidence patterns, potentially affecting younger males.
  • Grade 3 FL, particularly CD5-positive variants, may carry a risk of transformation to diffuse large B-cell lymphoma.
