Sus antecedentes:

  • La hemorragia subaracnoidea (SAH) es un subtipo de accidente cerebrovascular grave, generalmente causado por aneurismas intracraneales rotos.
  • La SAH afecta a las personas a una edad media de 55 años, lo que afecta significativamente a los años de vida productiva.

Objetivo del estudio:

  • Revisar el conocimiento actual y el tratamiento de la hemorragia subaracnoidea aneurysmal.
  • Identificar los desafíos y las direcciones futuras en la investigación y el tratamiento de la SAH.

Principales métodos:

  • Revisión de los ensayos clínicos existentes y estrategias de tratamiento para la hemorragia subaracnoidea.
  • Análisis de las tendencias de supervivencia, resultados del tratamiento y secuelas a largo plazo.

Principales resultados:

  • Las tasas de supervivencia para la hemorragia subaracnoidea aneurysmal han aumentado en un 17% debido a la mejora del diagnóstico, la reparación temprana, la nimodipina y la atención intensiva.
  • Los sobrevivientes con frecuencia experimentan deterioro cognitivo, trastornos del estado de ánimo, fatiga y trastornos del sueño, lo que afecta la función diaria y la calidad de vida.
  • Sólo dos intervenciones han demostrado mejores resultados en ensayos clínicos.

Conclusiones:

  • Los cuidados intensivos neurológicos especializados y la experiencia multidisciplinaria en centros de gran volumen son cruciales para el manejo de la SAH.
  • Los esfuerzos futuros deben centrarse en estrategias preventivas, mejoras en el cribado, técnicas menos invasivas de reparación de aneurismas y mejores evaluaciones de resultados, incluidas la función neurocognitiva y los biomarcadores.
